We are a full service home health agency providing you or your loved one with exceptional home health services. By centering our services around you, our registered nuses and therapists will develop a physician approved plan of care designed to meet your specific needs.

Home Health at Compass Park is licensed by the State of Indiana and is Medicare and Medicaid Certified. We are accredited by CHAP (Community Health Accreditation Program) with Deemed Status. Through “deeming authority” granted by the Centers for Medicare and Medicaid Services (CMS), CHAP has the regulatory authority to survey agencies providing home health, hospice and home medical equipment services, to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards. CHAP’s purpose is to define and advance the highest standards of community-based care.

We accept United Healthcare, Medicare, Medicaid, Anthem: Commercial, Managed Medicare/Medicaid, MHS Managed Medicare/Medicaid, CareSource: Commercial and Medicaid, and Private Pay clients.
